How to Use This Guide

Each agent entry below covers: mechanism of action (brief), administration guidance (fat or water soluble, food timing, form), standard dosing ranges used in integrative protocols, common side effects and how to manage them, contraindications, and key drug interactions. This guide is organized by agent class: macrocyclic lactones, benzimidazoles, nitroimidazoles, cestocides/trematocides, and azole antifungals. Botanical agents are covered separately in the second half.

Dosing ranges cited reflect integrative clinical practice — they are not official prescribing information. Drug Interaction Screening — Do This First

Before beginning any multi-agent antiparasitic protocol, run a complete drug interaction check using one or more of the following tools:

Enter every agent you plan to use — including botanicals, supplements, and any current medications — before beginning the protocol. Pay particular attention to interactions with anticoagulants (especially warfarin), immunosuppressants, CYP450-metabolized medications, and any agents with narrow therapeutic windows.

Pharmaceutical Agents — Per-Agent Reference

Ivermectin

Class: Macrocyclic lactone  |  Solubility: Fat-soluble  |  Take with: A high-fat meal to maximize absorption (studies show 2.5x increase in plasma levels with fatty food)

Standard integrative dosing: 0.2–0.4 mg/kg bodyweight per dose. In cycling protocols, typically administered once or twice weekly on treatment days rather than daily, to allow tissue distribution and avoid accumulation.

Common side effects: Mazzotti-like reaction (die-off/Herxheimer response) — fever, rash, edema, pruritus, myalgia — in patients with high parasitic burden. This is a detoxification response, not a drug reaction, and is managed by slowing the protocol pace and increasing drainage support. Mild GI symptoms (nausea, diarrhea) in a minority of patients.

Serious side effects: CNS toxicity (confusion, seizures, coma) is documented only in patients with blood-brain barrier compromise — specifically those with loiasis (Loa loa) microfilaremia or severe CNS pathology. In otherwise healthy individuals at standard doses, CNS events are exceedingly rare.

Contraindications: Known hypersensitivity. Use with extreme caution in patients with known or suspected Loa loa infection (microfilaremia). Avoid in children under 15 kg. Pregnancy: Category C — avoid unless clearly indicated.

Key drug interactions: Ivermectin is a P-glycoprotein substrate. Agents that inhibit P-gp (including some HIV protease inhibitors and azole antifungals) may increase Ivermectin plasma levels. Warfarin: no direct pharmacokinetic interaction documented, but monitor INR during any infectious/inflammatory treatment period.

Fenbendazole

Class: Benzimidazole  |  Solubility: Fat-soluble  |  Take with: A fat-containing meal. Absorption is significantly increased with dietary fat — take with avocado, olive oil, eggs, or any fatty food.

Standard integrative dosing: 222–444 mg/day, typically on a cycling schedule of 3 days on / 4 days off, or 6 days on / 1 day off depending on protocol. Extended or higher-dose use should be monitored with LFTs.

Common side effects: Generally very well tolerated at standard doses. Mild GI symptoms in some patients. Die-off reactions (Herxheimer-type) in patients with high parasitic load.

Serious side effects: Hepatotoxicity is documented with extended use at higher doses. Baseline LFTs and monitoring at 3–4 week intervals during extended courses are strongly recommended.

Contraindications: Known hepatic dysfunction — use with caution and close monitoring. Pregnancy: avoid — benzimidazoles are teratogenic in animal models.

Key drug interactions: Fenbendazole is metabolized by CYP450 enzymes. Concurrent use with other CYP450-metabolized agents may affect plasma levels of either drug.

Mebendazole

Class: Benzimidazole  |  Solubility: Poorly water-soluble; minimally absorbed systemically  |  Take with: A fatty meal to improve the modest absorption that does occur.

Standard integrative dosing: 100–500 mg/day in divided doses. Standard antiparasitic dosing ranges from 100 mg twice daily for 3 days to 500 mg twice daily for extended cycling protocols.

Common side effects: Minimal systemic side effects due to poor absorption. GI symptoms (cramping, diarrhea) in patients with heavy intestinal worm burden.

Serious side effects: Hepatotoxicity and agranulocytosis have been reported with extended high-dose use. LFTs and CBC monitoring are indicated for extended courses.

Contraindications: Pregnancy (teratogenic in animal models — Category C). Known hypersensitivity to benzimidazoles.

Key drug interactions: Cimetidine may inhibit mebendazole metabolism. Carbamazepine and phenytoin may decrease plasma levels. Metronidazole combined with mebendazole has been associated with rare cases of Stevens-Johnson syndrome — avoid this combination.

Albendazole

Class: Benzimidazole  |  Solubility: Fat-soluble  |  Take with: A high-fat meal — fat intake increases albendazole sulfoxide plasma levels up to 5-fold. This is a pharmacokinetic requirement.

Standard integrative dosing: 400 mg twice daily for systemic tissue-invasive indications. For neurocysticercosis and echinococcosis, 400 mg twice daily for 28-day cycles with 14-day rest periods between cycles.

Common side effects: GI symptoms (nausea, abdominal pain, diarrhea). Headache. Elevated liver enzymes. Hair loss with extended courses (reversible).

Serious side effects: Hepatotoxicity — monitor LFTs at baseline and every 2 weeks during extended courses. Bone marrow suppression — monitor CBC at 2–4 week intervals during extended use.

Contraindications: Pregnancy (teratogenic — Category C). Hypersensitivity to benzimidazoles. Significant hepatic dysfunction.

Key drug interactions: Dexamethasone and praziquantel increase albendazole sulfoxide plasma levels. CYP inducers (carbamazepine, phenytoin, rifampin) decrease plasma levels and may reduce efficacy.

Praziquantel

Class: Isoquinoline  |  Solubility: Fat-soluble  |  Take with: A fat-containing meal. Avoid grapefruit juice.

Standard integrative dosing: 25–50 mg/kg/day in 2–3 divided doses on treatment days. For tapeworm infections: 5–10 mg/kg as a single dose. For liver flukes and schistosomiasis: 25–40 mg/kg/day in divided doses for 1–3 days.

Common side effects: Nausea, abdominal pain, headache, dizziness, and drowsiness — typically transient. Die-off reaction may occur with high fluke or tapeworm burden. Do not drive on treatment days.

Serious side effects: In patients with neurocysticercosis, praziquantel can trigger seizures — requires corticosteroid co-administration and neurological monitoring. Do not use for ocular cysticercosis.

Contraindications: Ocular cysticercosis. Pregnancy: limited data — use only when clearly necessary. Caution in hepatic impairment.

Key drug interactions: CYP3A4 inducers (rifampin, carbamazepine, phenytoin, dexamethasone) markedly reduce plasma levels. CYP3A4 inhibitors (ketoconazole, itraconazole, cimetidine) increase plasma levels. Chloroquine reduces bioavailability.

Tinidazole

Class: Nitroimidazole  |  Solubility: Water-soluble; well absorbed orally  |  Take with: Food to reduce GI side effects. Avoid alcohol during treatment and for 72 hours after the last dose (disulfiram-like reaction).

Standard integrative dosing: For giardiasis: 2 g as a single dose. For amebiasis: 2 g/day for 3 days. For anaerobic bacterial overgrowth in cycling protocols: 500 mg twice daily on treatment days.

Common side effects: Metallic taste (very common), nausea, anorexia, GI discomfort. Mild CNS effects (drowsiness, headache, dizziness) in some patients.

Serious side effects: Peripheral neuropathy with prolonged use — discontinue if neurological symptoms develop. Transient leukopenia with extended courses.

Contraindications: First trimester of pregnancy (Category C — avoid). Known hypersensitivity to nitroimidazoles. Caution in CNS disease.

Key drug interactions: Warfarin: Tinidazole significantly potentiates warfarin anticoagulation via CYP2C9 inhibition — INR monitoring is essential. Lithium: may increase lithium levels. Alcohol: disulfiram-like reaction — strict avoidance required during and 72 hours after treatment.

Fluconazole

Class: Triazole antifungal  |  Solubility: Water-soluble; well absorbed orally  |  Take with: With or without food.

Standard integrative dosing: For Candida overgrowth in antiparasitic protocols: 150 mg once weekly or 100 mg daily for defined treatment periods. Systemic candidiasis: 200–400 mg/day.

Common side effects: Headache, nausea, abdominal discomfort. Rash (typically mild and transient). Elevated liver enzymes — monitor LFTs for extended courses.

Serious side effects: QT prolongation — significant concern when combined with other QT-prolonging agents. Hepatotoxicity with extended high-dose use. Stevens-Johnson syndrome (rare).

Contraindications: Concurrent use with QT-prolonging agents (amiodarone, quinidine, cisapride, astemizole). Pregnancy: Category D — avoid. Known hypersensitivity.

Key drug interactions: Fluconazole is a potent CYP2C9 and CYP3A4 inhibitor — significantly increases plasma levels of warfarin (INR monitoring essential), cyclosporine, tacrolimus, phenytoin, statins (especially simvastatin and lovastatin), and many other CYP-metabolized drugs. Rifampin significantly reduces fluconazole levels.

Itraconazole

Class: Triazole antifungal  |  Solubility: Fat-soluble  |  Take with: Capsule: take with a full meal or acidic beverage. Solution: take fasted for superior bioavailability. Capsule and solution are not interchangeable in dosing.

Standard integrative dosing: 100–200 mg once or twice daily. For extended fungal coverage in cycling protocols: 100 mg daily on treatment days. Pulse dosing (200 mg twice daily for 1 week per month) may be adapted for cycling antiparasitic contexts.

Common side effects: Nausea, abdominal discomfort, headache. Edema with extended high-dose use. Elevated liver enzymes — monitor LFTs.

Serious side effects: Negative inotropic effects — avoid in patients with heart failure. QT prolongation. Hepatotoxicity. Drug interactions are numerous and clinically significant.

Contraindications: Heart failure or ventricular dysfunction. Concurrent use with agents that depend on CYP3A4 for clearance with narrow therapeutic windows. Pregnancy: Category C — avoid.

Key drug interactions: Itraconazole is a potent CYP3A4 and P-gp inhibitor. Major interactions: statins (rhabdomyolysis risk — simvastatin and lovastatin contraindicated), warfarin (significantly increased INR), benzodiazepines (sedation), digoxin (toxicity), calcium channel blockers (edema, hypotension), cyclosporine, tacrolimus, sildenafil/tadalafil, and many others. A comprehensive interaction check is mandatory.

Botanical Agents — Administration & Practical Guidance

Black Walnut Hull (Juglans nigra)

Form: Tincture (green hull extract in alcohol), capsules, or powder. Tincture is preferred for bioavailability.  |  Take: On an empty stomach, 30 minutes before meals.

Dosing: Tincture: 20–30 drops in water, 2–3 times daily. Capsules: 500–1000 mg/day in divided doses. Dosing escalates over the first week before reaching maintenance levels.

Cycling: 3 weeks on / 1 week off during maintenance phase. Avoid extended uninterrupted use due to juglone's cytotoxic properties at high doses.

Cautions: Contains tannins — may interfere with iron absorption. Discontinue 2 weeks before elective surgery (mild anticoagulant properties). Nut allergy: theoretical cross-reactivity — use with caution in patients with tree nut allergy.

Wormwood (Artemisia absinthium)

Form: Tincture, capsules, or tea. Tincture is most commonly used.  |  Take: 30 minutes before meals.

Dosing: Tincture: 15–30 drops in water, 2–3 times daily. Capsules: 200–500 mg/day in divided doses.

Cycling: 3 weeks on / 1 week off. Wormwood contains thujone — a neuroactive compound with cumulative toxicity at high doses. Extended continuous use above recommended doses should be avoided.

Cautions: Thujone content may lower seizure threshold — avoid in patients with seizure disorders. Pregnancy: avoid (abortifacient properties historically documented). Warfarin: weak CYP interaction potential — monitor INR if used concurrently.

Cloves (Syzygium aromaticum)

Form: Freshly ground clove powder (not pre-ground — eugenol content degrades rapidly), capsules, or tincture.  |  Take: With or immediately after meals to reduce GI irritation.

Dosing: Powder: 500 mg–1.5 g/day in divided doses. Fresh-ground is strongly preferred.

Cycling: 3 weeks on / 1 week off. As part of the Clark triad, taken simultaneously with black walnut hull and wormwood for synergistic life-cycle coverage.

Cautions: Eugenol has anticoagulant properties — use with caution on warfarin or other anticoagulants. High doses of eugenol are hepatotoxic in animal models — stay within recommended dose ranges.

Oregano Oil (Origanum vulgare)

Form: Enteric-coated softgels strongly preferred — they protect gastric mucosa and deliver active compounds to the small intestine.  |  Take: With meals.

Dosing: 50–150 mg of standardized carvacrol content per dose, 2–3 times daily. Aim for minimum 70% carvacrol content for therapeutic activity.

Cycling: 3 weeks on / 1 week off. Oregano oil significantly disrupts gut microbiome at therapeutic doses — cycling prevents overshoot dysbiosis.

Cautions: Anticoagulant properties — caution with warfarin and antiplatelet agents. Carvacrol may reduce non-heme iron absorption — separate from iron supplements by at least 2 hours. Pregnancy: avoid at therapeutic doses.

Mimosa Pudica Seed

Form: Capsules or powder (forms a gel when mixed with water).  |  Take: On an empty stomach, 30–60 minutes before meals.

Dosing: 500 mg–2 g/day in divided doses. Start low and increase gradually — rapid introduction can cause temporary constipation. Increase water intake significantly.

Cycling: Can be used continuously during the pharmaceutical phase and botanical maintenance. Continuous use is generally well tolerated due to its mechanical primary mechanism.

Cautions: Constipation risk if water intake is inadequate — minimum 2–3 liters/day required. May bind medications in the GI tract — take any oral medications at least 2 hours away from doses. Pregnancy: limited safety data — avoid.

Papaya Seed

Form: Fresh seeds (preferred), air-dried seeds, or capsules.  |  Take: With honey (as used in the Okeniyi et al. RCT documenting 76.7% parasite clearance) or mixed into food.

Dosing: 1–2 tablespoons of fresh seeds per day, or capsule equivalent (typically 500–1000 mg/day of dried seed powder).

Cycling: Can be used daily throughout the maintenance phase.

Cautions: Carpaine is a cardiac alkaloid — therapeutic doses are far below bradycardia threshold seen in animal models. Pregnancy: avoid (emmenagogue and abortifacient properties). Male fertility: possible temporary reduction in sperm motility at high doses.

Modified Citrus Pectin (MCP)

Form: Powder (dissolved in water or juice) or capsules.  |  Take: Can be taken with or without food. For heavy metal and toxin binding: take away from meals when possible.

Dosing: 5–15 g/day in divided doses. Minimum 5 g/day throughout the pharmaceutical phase and botanical maintenance. Higher doses (up to 15 g/day) used in oncology-adjacent contexts or when heavy metal detoxification is a primary goal.

Cycling: MCP is used continuously throughout all phases — it does not require cycling. MCP's primary mechanisms (galectin-3 binding, toxin chelation) do not carry cumulative toxicity risk at recommended doses.

Cautions: Citrus allergy: theoretical concern — use with caution in patients with known citrus hypersensitivity. GI sensitivity: start at 5 g/day and increase gradually over 1–2 weeks. Take oral medications at least 1–2 hours away from MCP doses.
