At Diagnosis

Understanding Your Diagnosis

  • What specific type of MS/TM/NMOSD do I have, and what does that mean for my prognosis?
  • Were all alternative diagnoses ruled out? What tests confirmed this diagnosis?
  • Should I have AQP4-IgG and MOG-IgG antibody testing if I haven't already?
  • How active is my disease right now based on my MRI and clinical findings?
  • What does my lesion burden and location suggest about my risk of disability progression?

Starting Treatment

  • What DMT do you recommend for me, and why this one over others?
  • What is the efficacy of this DMT in reducing relapses and slowing progression?
  • What are the most common and most serious side effects?
  • What monitoring will I need (blood tests, MRI, eye exams)?
  • How long before I know if it's working?
  • What happens if it doesn't work — what's the next step?

At Follow-Up Appointments

Disease Monitoring

  • Has my MRI shown any new or enlarging lesions since my last scan?
  • Has my disability level changed based on your examination today?
  • Are there any signs my current DMT is not working effectively?
  • When is my next MRI scheduled, and what are we looking for?

Symptom Management

  • I've been experiencing [specific symptom] — what's causing it and what can we do?
  • Is this a new symptom or a relapse? How do we tell the difference?
  • Are there medications or therapies that could help with [fatigue/spasticity/bladder/pain/cog fog]?
  • Should I be referred to any other specialists for this symptom?

Lifestyle and Integrative Care

  • What vitamin D level should I be targeting, and what dose do you recommend?
  • Are there any supplements I should avoid given my current DMT?
  • Can you refer me to a neurological physiotherapist / dietitian / neuropsychologist?

If You're Considering Changing Treatment

  • Why are you recommending a change in my DMT?
  • What are the risks of switching versus staying on my current therapy?
  • Is there a washout period required, and what are the risks during that time?
  • Are there any clinical trials I might be eligible for?

If You Have NMOSD Specifically

  • Am I AQP4-IgG positive or negative, and how does that affect my treatment options?
  • Which NMOSD-specific therapies are appropriate for me?
  • Are there any MS medications I must absolutely avoid?
  • How quickly should I seek care if I think I'm having an attack?

If You're Planning a Pregnancy

  • Is my current DMT safe during pregnancy and breastfeeding?
  • Is a washout period required before conception, and how long?
  • What is my relapse risk during pregnancy and postpartum?
  • Who should be on my care team during pregnancy?

Questions About Your MRI

  • Can I see my MRI images and have you walk me through what you're seeing?
  • Where are my lesions located, and what functions do those areas control?
  • Has there been any brain atrophy since my last scan?
  • What would trigger a change in my treatment based on future MRI findings?

Red Flags — When to Seek Immediate Care

  • Sudden vision loss in one eye
  • Rapid onset weakness or paralysis
  • Loss of bladder or bowel control appearing suddenly
  • Severe balance problems or inability to walk
  • High fever with worsening neurological symptoms
